A healthy 22-year-old man in Jordan went in for a routine operation on his varicoceles, enlarged veins in the scrotum that were causing him symptoms. He had never had abdominal or pelvic surgery. There was no reason to expect anything unusual inside.
During the bilateral laparoscopic procedure, surgeons ran into dense adhesions where the sigmoid colon had stuck over the left internal inguinal ring, and had to separate them carefully to reach the spermatic vessels. While they were working, a smooth, egg-shaped object came into view in the left paracolic gutter, the channel running alongside the colon.
