Multiple failures have been identified in the way Edinburgh City Council carries out recommendations arising from whistle-blowing disclosures, raising concerns staff could lose faith in the system.
Shock findings of an annual audit revealed that in one case the wording of a recommendation was changed by an official.
The report, presented to the Governance, Risk and Best Value Committee on Tuesday (October 11) highlighted a range of issues including a lack of oversight over the implementation of whistle-blowing recommendations, instructions not fully being followed and failure to retain evidence of changes being made.
