Federal inspectors who spent six days inside a Houston compounding pharmacy in August documented 14 problems with how it made sterile injectable drugs, including compounded GLP-1 weight-loss shots, according to an FDA inspection report. The pharmacy, Houston RX LLC, later recalled 64 vials of compounded tirzepatide with niacinamide because of a lack of assurance of sterility.
The findings matter to anyone who buys compounded semaglutide or tirzepatide through a telehealth program, clinic, or med spa. Injectable drugs bypass the skin's natural barrier, so a contaminated vial can cause infection at the injection site or, less often, in the bloodstream. Patients usually cannot tell a safe vial from a compromised one by looking at it. Equipment that has not been depyrogenated, a process that removes fever-causing bacterial residues called endotoxins, can leave those residues behind even when no live germs remain.
