Told to Give Up Seafood for Life, 86% of These Patients Ate Cooked Fish Without a Single Symptom

Someone eats sashimi, develops hives and breathing trouble within hours, tests positive for antibodies to a fish parasite, and is told to stop eating seafood. Not raw seafood. All of it, including broth.

New research from a Japanese allergy clinic suggests the advice is often wrong. Among 44 outpatients with Anisakis allergy, 86.4% tolerated cooked fish and 95.5% tolerated seafood broth without symptoms. The study comes from Showa Medical University and appears in Allergology International.

The Worm That Keeps Causing Trouble After It Dies

Anisakis is a marine roundworm whose larvae end up in raw or undercooked fish and squid. Swallow a live one, and it can attach to the wall of the esophagus, stomach, or intestine, causing abdominal pain, nausea, vomiting, distention, and diarrhea. Humans are a dead end for the parasite, so it eventually dies and provokes an inflamed mass.

The allergy problem is separate, and it is the reason patients are told to avoid everything. Several of the worm's proteins survive treatments that reliably kill the animal. A study of 27 sensitized patients found that 81% had IgE to pepsin-resistant allergens and 67% to heat-resistant ones, and separate work has identified allergens that survive boiling.

The European Food Safety Authority summarized the concern in December 2025, noting that thermostable proteins including Ani s 1 and Ani s 7 remain active after standard freezing or cooking and can drive reactions ranging from hives and angioedema to severe anaphylaxis.

Given that chemistry, blanket avoidance looks like the cautious call. The new data suggest that caution and accuracy have drifted apart.

Forty-Four Patients and an Unexpected Result

The team conducted a single-center retrospective cohort study of 44 Anisakis allergy outpatients, classifying them based on their actual responses to heated fish and broth rather than on antibody levels alone. The mean age was 47, and in 40 of the 44 cases, anaphylaxis had occurred within 12 hours of consuming fresh seafood.

Patients who ate cooked fish and broth without symptoms were classified as a gastroallergic anisakiasis-like type. Those who reacted were classified as having a classical Anisakis allergy-like reaction and underwent a stepwise oral fish challenge followed by tolerability induction.

Thirty-eight of the 44 tolerated cooked fish, and 42 tolerated broth. Among the 38, all 30 who tried thawed frozen fish tolerated it too. Only six patients fell into the classical type, all with mild-to-moderate reactions, and five of them were eventually able to eat fish without symptoms under the protocol.

One finding runs in the opposite direction and deserves attention. Severe anaphylaxis occurred in 39.5% of the group that tolerated cooked fish. Tolerating cooked fish, in other words, says nothing about how dangerous the original raw exposure was.

Two Kinds of Reaction Hiding Under One Label

The study builds on a distinction proposed by earlier researchers. Gastroallergic anisakiasis is triggered by living larvae actively invading tissue. True Anisakis allergy involves reactions even to nonviable parasite material, meaning cooked or frozen fish can trigger it.

If that framework holds, the diagnostic question is not simply whether a patient has Anisakis-specific IgE. It is unclear which of the two conditions produced their reaction, and the two look identical on a routine antibody test.

Important caveats apply. This was a retrospective study at a single center, involving 44 patients in a country where raw fish consumption and Anisakis exposure are unusually high. The oral food challenges were conducted under medical supervision. Nobody who has had anaphylaxis after eating fish should reintroduce it based on a news article. That is a conversation for an allergist, with a plan.

How Many People Are Sensitized Without Knowing

The scale of quiet exposure may be considerable. The European assessment describes a rising number of people who develop sensitivity to thermostable Anisakis antigens without obvious symptoms, often after a mild or undiagnosed gastric infection, and it ranks anisakiasis among the top 10 of 24 parasitic foodborne infections in Europe, with clinical cases frequently misdiagnosed as ordinary gastrointestinal complaints.

The numbers behind that concern are striking. In one Spanish study, 51 of 403 blood donors were sensitized to the parasite, and 47 of them had no symptoms. A systematic review of sensitization studies found rates in the general population ranging from 0.4% to 27.4%, varying widely by region and testing method.

For US consumers, the preventive advice remains unchanged. The CDC points to FDA guidance on freezing fish intended to be eaten raw, at minus 4 degrees Fahrenheit or below for seven days, or minus 31 degrees until solid with specified holding times, and cooking seafood to an internal temperature of at least 145 degrees Fahrenheit. Those steps kill the parasite. What the new research clarifies is what killing the parasite does and does not mean for people who have already reacted to it.
