American Regent has issued three nationwide injectable recalls in roughly six weeks, and the same material turns up in every one of them. Glass.
The most recent action, announced September 3, covers three lots of Epinephrine Injection, USP 30 mg per 30 mL multiple dose vials after customer complaints of leaking and cracked vials. The company's investigation also revealed particulate matter identified as nylon, cellulosic material, acrylic, polyethylene, and glass. Six weeks earlier, the same manufacturer recalled a lot of papaverine hydrochloride injection over glass and paraformaldehyde particles. A week after that, its animal health division recalled four lots of Adequan injections for dogs and horses after glass fibers were found in retained samples.
