On Wednesday, a watershed moment for maternity safety will take place as the final report of the inquiry into the largest maternity scandal ever seen within the NHS is set to reveal failings in the care of hundreds of women and babies.
In 2019, The Independent revealed that an inquiry into maternity care at Shrewsbury and Telford Hospital NHS Trust had found that more than a dozen women and more than 40 babies had died during childbirth because of a culture that failed to listen to parents.
