The share of commercially insured Americans diagnosed with opioid use disorder climbed 32.7% from 2022 to 2025, rising from 449 to 596 patients per 100,000, according to FAIR Health's annual Opioid Tracker update released Oct. 14. Tennessee had the highest rate at 1,551 per 100,000, about 2.6 times the national figure. West Virginia, Mississippi, Arkansas, and Delaware followed.
The numbers count diagnoses recorded on private insurance claims. They do not count everyone who misuses opioids, and a rise can partly reflect more people being screened, diagnosed, and treated.
