Medical mistakes led to grandmother's death: coroner

A "catastrophic" series of errors by medical staff during and after a surgical procedure led to a 48-year-old grandmother's death, a coroner has found.

The "chain of tragedy" that resulted in Christine Guyula's death at Royal Darwin Hospital on March 17, 2022 has prompted a coroner to recommend changes to anaesthesia rules.
