A Nottinghamshire mum's medication mistake was the cause of her death which robbed her three children of the "young and vibrant" parent. Charlotte Fisher-Gibson, 33, from Forest Town, Mansfield, died July 15, 2022 leaving behind her children Faith, William, and Maddison, who were 13, 11 and nine-years-old at the time.
It was her daughter Faith who came down the driveway of their house on Friday, July 15 to discover what had happened. After calling paramedics, she tried her best while listening to emergency services over the phone to save her mum's life, however it was too late.
An inquest opened on December 22, 2022 and on June 12, 2023 was concluded by coroner Laurinda Bower at the Council House in Old Market Square, Nottingham. The coroner explained the mum's sudden death, stating she had died as a result of acute mixed drug toxicity.

She had also inhaled carbon monoxide, which could have contributed to her death. Miss Bower gave the verdict of a narrative conclusion.
"Charlotte Fisher-Gibson sadly died on July 15, 2022, at her home address in Forest Town, Nottinghamshire, at some time after 11am but before 3pm hours," the coroner said.
"She died as a result of acute mixed drug toxicity. The inhalation of carbon monoxide probably more than minimally contributed to Charlotte’s death, but I have been unable to ascertain the probable source of the carbon monoxide."
Maria Bough, Charlotte's mum, speaking after the inquest, said she had mistakenly taken too many painkillers. She explained the loss meant she was now supporting her grandchildren in order to give them the life Charlotte planned for them.
"We have three children now growing up without their Mum. Charlotte was a young, vibrant woman with everything to live for," Ms Bough said.
"The acute mixed drugs toxicity was brought on simply by taking a variation of an analgesic medication (painkiller) that all basically did the same job. Ignorantly she took them to relieve pain.
"Sadly, these shut down the part of the brain that dealt with Charlotte's heart and lungs. The pathologist who did Charlotte's post mortem said that the toxic range of carbon monoxide in her bloodstream, would have made her quite sedated and confused. It is now my job to try and find out where/ how the carbon monoxide got into her bloodstream.
"In all this we have to now look at finding a way forward to build her children the future my beautiful daughter worked so hard to give them - by finding justice for how she died the way she did."
