The Thirlwall Inquiry has found a 'complete failure' to protect babies from Lucy Letby, concluding in its 15 September 2026 report that earlier action by hospital leaders might have saved three babies and protected seven others from harm.

Lady Justice Kathryn Thirlwall blamed serious failures in safeguarding and hospital leadership, saying staff needed only suspicion of deliberate harm to begin safeguarding action.
