Living Within a Tenth of a Mile of a Dry Cleaner Was Linked to Higher Parkinson's Odds in a Nationwide Medicare Study

People who lived within 500 feet of a dry cleaner had 14 percent higher odds of being diagnosed with Parkinson's disease than people living four to five miles away, according to a nationwide analysis of Medicare records published in the journal npj Parkinson's Disease.

The pattern held across distance. Odds were highest for those closest to a dry-cleaning business and declined steadily as residential distance increased, a gradient that researchers treat as one signal that an association may reflect something real rather than statistical noise.

The study is large. It included 180,982 people with newly diagnosed Parkinson's disease and more than 18.8 million control participants living within five miles of a dry cleaner. It is also, by design, unable to prove that solvents caused anyone's disease.


The Finding and Its Distance Gradient

Researchers used Medicare data from 2014 through 2018, identifying new Parkinson's cases among beneficiaries aged 67 and older. They located 32,723 dry-cleaning establishments and measured the distance from the nearest one to each participant's residential address as of two years before diagnosis or a matched reference date.

The analysis adjusted for age, sex, race, smoking probability, health care utilization, neighborhood income, rural and urban classification, and area-level fine particulate air pollution.

Associations were stronger for certain clinical patterns recorded in claims. Among people living within a tenth of a mile of a dry cleaner, odds were 23 percent higher for Parkinson's cases with prior gait disturbance codes, 25 percent higher for cases with dementia or cognitive decline codes, and 19 percent higher for cases with prior falls. No significant association appeared for the claims-defined tremor group, though the authors cautioned that tremor may be poorly captured in administrative billing data. Those clinical groups were not mutually exclusive.

The relationship appeared in both men and women, though it was stronger and extended over greater distances in men. Among women, significant associations were largely limited to the two nearest distance categories. Geographic mapping showed the pattern was not confined to one region, with stronger associations appearing around the Dallas-Fort Worth and Detroit metropolitan areas, southern Illinois, the Tennessee-Kentucky border, central Pennsylvania, north-central Indiana, parts of southern California, and the New York City and New Jersey area.


The Chemistry That Made This Question Worth Asking

Genetic factors are thought to explain fewer than one in ten Parkinson's cases, which has pushed researchers toward environmental exposures.

Two chlorinated solvents historically used in dry cleaning have long been studied as neurotoxicants: trichloroethylene, known as TCE, and perchloroethylene, also called PCE or perc. Both are small molecules that distribute readily in the brain, and laboratory research has connected TCE to loss of dopamine-producing neurons in animal models. Both persist in soil and groundwater for long periods, which is why contamination concerns extend beyond the buildings where they were used, as the study summary notes.

Regulators have already acted on the hazard, though the practical status of that action is unsettled. The Environmental Protection Agency finalized a rule in December 2024 prohibiting TCE uses on staggered phase-out timelines. The agency has since revised several compliance deadlines, and in May it postponed the effective date of conditions attached to certain exempted uses until federal litigation over the rule concludes. A companion rule covering perchloroethylene, finalized the same month, also had several compliance dates extended in July.

The Limits Are Substantial and Stated Plainly

The authors were direct about what their data cannot show, and readers should be too.

This is a case-control study built on administrative claims. It can identify associations. It cannot establish causation. Distance to a dry cleaner served as a proxy for potential exposure because the researchers did not measure any individual's actual TCE or PCE exposure. The business classification used may include locations that only accept and return garments without performing cleaning or using solvents on site.

Dry-cleaner locations came from 2013 data, and residential addresses were assessed two years before diagnosis, so long-term residential stability had to be assumed. The researchers could not rule out unmeasured neighborhood characteristics or co-exposures that travel with proximity to commercial strips. Findings in Medicare beneficiaries aged 67 and older may not generalize to younger populations.

Living near a dry cleaner is also correlated with living in denser commercial areas, which carry other exposures. The adjustment for fine particulate pollution addresses part of that concern but not all of it.

Nothing in this study changes clinical practice, and there is no screening test for Parkinson's risk to request. No medical organization has issued guidance based on it.


Practical Perspective for People Who Live Near One

The reasonable response to this finding is interest, not alarm. A 14 percent relative increase in odds applied to a condition that remains uncommon translates to a small absolute difference for any individual.

Nobody should move, and no one should read a new tremor as a diagnosis. Parkinson's symptoms, including tremor at rest, slowed movement, muscle stiffness, and balance problems, warrant evaluation by a clinician, ideally a neurologist, regardless of where a person lives. Those symptoms have many causes, and several are treatable.

People with a specific concern about a known contaminated site near their home can contact their state environmental agency or check whether the location appears in federal cleanup databases. Households on private wells near former industrial or dry-cleaning operations can ask about volatile organic compound testing through their county health department, which sometimes offers it at reduced cost.

What happens next is replication. Studies of this kind gain weight when independent groups reproduce them using different populations and, ideally, measured exposure rather than distance proxies. Whether the EPA's solvent restrictions survive litigation and take effect on their current schedule is a separate open question with its own timeline.
