Multiple red flags were missed by child protection authorities before a five-year-old girl's death that shocked the nation, a damning report says.

Sweeping changes have been recommended for the Northern Territory's child protection system after a review found workers repeatedly failed to adequately assess safety threats to Kumanjayi Little Baby's family.
