The Ghost is an anomaly in Animal Hospital, and it can damage the player for 2 damage per tick just by touching him. It can also toggle invisibility on and off as well as open and close all of the doors in the hospital. You can either deal with it by killing or incapacitating it. To kill it, you must sneak behind it while it is visible and use a gun or a taser on it. Incapacitating it will force you to either trick it into following you into a fire or spraying it with a fire extinguisher which will make it always be visible.
