A long-awaited report into a fatal sinking has concluded the shipping vessel had been operated in an "unsafe manner".

The 58-page study by the Marine Accident Investigation Branch (MAIB) set out in great detail the circumstances that led to the deaths of Carl McGrath, Ross Ballantine and Alan Minard. The report documented a series of operational failings as the boat's captain, Mr McGrath, sought extra productivity in his bid to buy the vessel outright.
