A woman died after being discharged from hospital.
The family of Barbara Osbaldeston are now searching for answers, after she died at home. An investigation has been launched, after the 64-year-old's family raised concerns about her death.
Barbara, who was originally from Southport but living in Preston, was found dead at her home in Lea on November 9, last year. Now an inquest will aim to find out which organisation was responsible for Barbara's care after being discharged from hospital.
