Cardboard Found Inside a Sealed Bag of Sterile Antibiotic Prompts a Nationwide Baxter Recall

A piece of cardboard was found floating in a sealed bag of sterile antibiotic solution meant to be infused directly into a patient's bloodstream.

Baxter International is voluntarily recalling one lot of Cefazolin in Dextrose Injection, USP, 2 grams per 100 milliliters, nationwide, after a report of particulate matter in the solution that the company identified as cardboard. The affected lot is LD175708, expiring February 14, 2027, supplied as a frozen premix in a 100 milliliter plastic container.

Baxter said the defective product carries a "reasonable probability of causing pulmonary emboli," along with blockages in other blood vessels that can lead to tissue death and organ damage, and phlebitis. The company added that foreign particles infused intravenously can trigger systemic immune activation, organ dysfunction, and the breakdown of red blood cells.

No adverse events have been reported.


Why the Contaminant Matters More Than the Word Particulate

Recall notices flatten everything into the phrase particulate matter, which obscures what is actually being described.

Glass fragments in an injectable, the subject of a separate recall this month, point to a container failure. Cardboard points somewhere else entirely. Cardboard is not a manufacturing input, a byproduct, or a degradation product. It is packaging. Its presence inside a sealed sterile solution means material from the outside of the process reached the inside of the product.

Sterile injectable manufacturing is built specifically to prevent that. Premix solutions are compounded and filled in classified cleanroom environments where air is filtered and pressurized, personnel are gowned, materials pass through airlocks, and shipping cartons are ordinarily excluded from filling areas precisely because fibrous packaging sheds particles. Filled containers then pass visual or automated inspection intended to catch visible foreign matter before release.

Cardboard reaching a sealed bag and surviving to distribution means more than one of those layers did not catch it.


What Can and Cannot Be Said About Accountability

Here the reporting has to stop short of where the question naturally leads, and it is better to say so than to imply more than is documented.

Baxter's notice does not name the facility that produced the lot, does not describe how the cardboard entered the product, and does not state what corrective action was taken. What is on the public record is that Baxter has issued prior particulate recalls. The FDA has posted a 2017 recall of one lot of Nexterone, an amiodarone premix, later expanded to a second lot, over potential particulate matter that the company said may have entered during manufacturing. The agency has also posted a Baxter recall of two lots of intravenous solutions in which the particulate was determined to be an insect, identified through a customer complaint. Both of those recalls have since been terminated.

Those are individual, resolved events across a company that produces an enormous volume of intravenous products, and stringing them together does not establish a pattern of failure. They do show that foreign material has previously reached sealed Baxter solutions and that customer complaints, rather than internal detection, surfaced at least one of them. The current recall also began with a report rather than with routine screening.

The FDA had not published a classification for this recall when this article was written. Classification indicates the agency's own hazard assessment, and its absence means the company's risk language stands as the operative description.


Who Was Actually Exposed

The practical exposure here is narrower than the risk language suggests, and readers deserve that context rather than alarm.

Cefazolin is a workhorse antibiotic given intravenously to treat bacterial infections and, most commonly, as prophylaxis before surgery. Almost anyone who has had an orthopedic, cardiac, abdominal or cesarean procedure in a U.S. hospital has likely received it. It is not a product any patient keeps at home, and no consumer can check a cabinet for it.

The timing narrows things further. The recalled lot was distributed across the United States between March 25 and June 30, 2025, more than a year before the recall. Much of a hospital's stock from that window has almost certainly already been used, which means this recall is substantially retrospective. Patients who received the affected lot would have been treated in 2025 and would have experienced any consequence at that time.

The patients who would have been most vulnerable to an infused particle are those with existing cardiovascular or pulmonary compromise, and anyone receiving the infusion without an in-line filter. In-line filtration is common in many settings but not universal.

Baxter has instructed healthcare facilities to stop using the affected lot immediately and return the product. No other lots are affected.


What Patients Should and Should Not Do

For nearly everyone reading this, the correct action is none.

There is no test for past exposure to an infused particle, no screening that would be informative now, and no reason to seek evaluation because of surgery in 2025. Someone who developed an unexplained pulmonary embolism, unexplained vein inflammation or organ problems in the weeks following an infusion in that window could reasonably mention it at their next appointment, but coincidence is a far more likely explanation than this lot.

Nobody should stop or delay a prescribed antibiotic because of this recall. Cefazolin remains widely used, and a single recalled lot does not reflect on the drug.

Patients scheduled for surgery who want reassurance can ask whether the facility has verified its inventory against the recalled lot number. Hospitals receive these notices directly and act on them as routine practice.
