Two men and two teens were taken to Nepean Hospital but paramedics were unable to save the woman. (AAP)

A woman has died after a boat with five people on board capsized on the Nepean River in western Sydney.

Emergency services were dispatched after reports a boat had overturned after noon on Wednesday.

Fire and Rescue NSW said the tinny capsized just after 12.30pm after the occupants unintentionally went over the Penrith Weir.

Two men and two teenagers were rescued from the water and taken to Nepean Hospital for observation and treatment of minor injuries.

Police said a woman was pulled from the water unconscious and treated by paramedics but died a short time later.

She is yet to be formally identified but is believed to be aged 33.

"This is a tragic ending for this family on what should have been a fun day out on the water this Australia Day," NSW Ambulance Acting Superintendent Greg Marshall said.

"This is a reminder to everyone out on the water this summer on how quickly things can turn".
