Three people ordered draft beer at three different taverns in the same U.S. county and got something far more dangerous. In each case, a caustic cleaning solution left in the tap lines ended up in the glass, and months later they were still dealing with esophageal damage, altered taste, and trouble swallowing.
The cases are described in a report published in February in Case Reports in Gastroenterology by authors from the Kirk Kerkorian School of Medicine at the University of Nevada, Las Vegas, and the University of Nevada, Reno. The authors, who thanked the Southern Nevada Health District for its collaboration, call the hazard largely unrecognized and blame gaps in regulatory oversight and enforcement.
A Cleaning Solution That Should Never Reach a Glass
Draft systems need regular cleaning. The authors note that the Brewers Association recommends alkaline cleaning every two weeks and acid cleaning every three months, each followed by a thorough water rinse.

Three Patients with Injuries That Lingered
The patients, all from the same county and seen at the same clinic over a three-year period, were involved in unrelated incidents.
A previously healthy 37-year-old woman felt immediate burning in her mouth and esophagus, followed by persistent vomiting, heart palpitations, reflux, and burning diarrhea every day for two weeks. Three months later, she still had nausea, burning, and a metallic taste when using silverware. An endoscopy found redness in the lower esophagus and a 2-millimeter nodule higher up. She was told to avoid metal utensils and scheduled for yearly surveillance scopes. "It feels like I've eaten many hot peppers, and I can't use metal silverware because everything tastes metallic and causes tingling in my mouth," she said at a follow-up visit.
A previously healthy 37-year-old man swallowed about 3 ounces and immediately felt his tongue and esophagus burn. He described the taste as "an acid banana." Bitters and tonic offered by staff made things worse, triggering vomiting, abdominal pain, and diarrhea. Poison Control recommended limited intervention, and he treated himself with water and high-dose omeprazole. Endoscopies at one and 10 months showed inflammation under the tongue, esophageal ulcers and stomach inflammation. He still has trouble swallowing and altered taste and needs long-term acid-suppressing medication.
A 40-year-old man who took a single gulp sought care a month later. He described an immediate "fire" sensation, followed by vomiting, mouth burns, and voice changes, and he later lost his sense of taste and smell and lost weight. His first endoscopy showed esophagitis and a benign narrowing where the esophagus meets the stomach. Follow-up found a hiatal hernia and Grade D esophagitis, the most severe grade, with breaks in the lining covering at least 75% of the esophagus's circumference. He remains on high-dose medication and nutritional support and has other conditions, including chronic fatigue syndrome and fatty liver disease.
Not the First Time Line Cleaner Has Been Served
The authors cite news reports of similar incidents in the United Kingdom and several U.S. states. In 2023, Fox8 reported that cleaner left in the beer lines at a Winston-Salem, North Carolina, pub sickened three people and could be to blame for the death of one man, according to his friends. The authors describe it as a possible fatality.
Southern Nevada has seen a high-profile case too. In 2022, a Clark County jury awarded $8 million to a Henderson man who was served chemical cleaning fluid instead of a beer sample at a casino brewery in 2018, KLAS-TV reported. The case report does not identify its patients or tie them to that lawsuit. Its authors note that there is ongoing litigation related to their cases, but they say no information from it was used.
The authors also point to a 2024 conference abstract describing a 33-year-old Massachusetts woman who developed esophageal ulcers after drinking a contaminated shot and continued to notice a metallic taste.
In the U.K., they note, some pubs follow standardized line-cleaning charts and train staff on cleaner safety, and the British Institute of Innkeeping recommends testing beer with litmus paper before serving it.
Calls for Dyes, Testing and Better Tracking
The authors urge standardized flushing protocols, tracer dyes and point-of-use testing to confirm that what comes out of the tap is really beer. They also want better tracking, noting that without a national reporting system, the true number of these injuries is unknown.
The report has clear limits. It includes only three patients, and the specific cleaning product and amount involved could not be determined.
Caustic burns can have lasting effects. The MSD Manual notes that such burns can perforate the esophagus or stomach, and that burns that do not perforate can scar the esophagus. The case report authors note that endoscopy is recommended within 24 to 48 hours of an accidental caustic ingestion to assess the damage.
