The indictment alleges at least $19 million in fraudulent claims, which led the insurer to pay more than $4.8 million to All Types Counseling Services.
All allegations come from the indictment as described by the U.S. Attorney's Office for the Northern District of Illinois. The defendants are presumed innocent, and prosecutors noted that "an indictment is not evidence of guilt."Arraignments had not been scheduled as of the announcement.
