A 65-year-old sex offender took his own life at HMP Wormwood Scrubs despite officers knowing he was suffering from paranoia and worsening mental health.
Peter Magloire, who believed he was under threat from prisoners and staff he claimed knew about his historic offences, was found unconscious from asphyxiation in his cell in January 2023. He died in hospital several days later.
