Until September 3, every federally cleared option against New World screwworm in pets could only treat an infestation that had already started. Now there is one that can prevent it.
The Food and Drug Administration issued an Emergency Use Authorization for Capstar tablets and revised an existing authorization for generic nitenpyram tablets, clearing both for prevention as well as treatment of New World screwworm infestations in dogs, puppies, cats and kittens. The animals must weigh at least two pounds and be at least four weeks old. The generic tablets were authorized in June for treatment only.
Timothy Schell, PhD, director of the FDA's Center for Veterinary Medicine, said in the agency's announcement that multiple products had already been authorized or conditionally approved for treatment but none for prevention. He described the action as one in which "the FDA is addressing a previously unmet need."
New World screwworm is a parasitic fly whose larvae feed on living tissue rather than dead tissue. A female lays eggs in open wounds or around body openings, and the maggots burrow into healthy flesh. Untreated infestations cause severe damage and can kill the animal.
The Risk Is Tied to Where an Animal Has Been
This is not a nationwide pet health emergency, and framing it that way would misrepresent the situation.
The United States eradicated the parasite decades ago using the sterile insect technique, and eliminated a small outbreak in the Florida Keys in 2017. What changed is that cases have moved northward through Central America and into Mexico over the past several years, and detections have since been confirmed again in the United States. The FDA says confirmed domestic cases remain within a limited geographic region, so most dogs and cats in the country are at low risk.
Because that picture can change and because case dashboards update independently of this authorization, owners who want current numbers should consult the U.S. Department of Agriculture's dashboard of confirmed detections rather than relying on a figure quoted in any news article, including this one. The federal response is coordinated through a joint government website.
The households with a real reason to pay attention are specific: people relocating pets across the southern border, those traveling with animals to affected regions, people importing or adopting animals from Mexico or Central America, and those living in or near areas where infestations have been identified. For a dog that lives in Chicago and never leaves, this authorization is background information.
Reading the Label Before Reaching for the Bottle
Both products are available over the counter in two tablet sizes, 11.4 mg and 57 mg, with dosing based on the animal's weight.
That over-the-counter availability creates a specific hazard worth naming. Nitenpyram has been sold for flea control for years under the Capstar brand, which means many households already have a bottle in a cabinet. The temptation to dose a pet based on a general sense that it treats parasites is exactly the wrong approach here, because the authorized use for screwworm has its own fact sheet with its own instructions.
The FDA directs pet owners to review the relevant product's fact sheet before use. Fact sheets, authorization letters, and summaries are posted on the agency's screwworm authorization page, alongside a broader list of animal drugs for screwworm. Do not give either product to an animal under two pounds or younger than four weeks old.
An emergency use authorization is also not the same as full approval. The agency concluded it is reasonable to believe the products may be effective for this use and that the known and potential benefits outweigh the known and potential risks. That is a lower evidentiary bar than standard approval, and the authorizations remain in effect until they are revoked or until the Secretary of Health and Human Services terminates the underlying emergency declaration.
Signs That Warrant a Veterinarian Rather Than a Tablet
Prevention is a different situation from an active infestation, and the second one is a veterinary emergency.
Owners should look for open wounds that appear to be worsening rather than healing, visible maggots in or around a wound or a body opening such as the eyes, ears, nose or mouth, a foul odor, obvious pain or agitation, and loss of appetite. Wounds from recent surgery, tick removal, fighting, ear tagging, or even a fresh navel in a newborn animal are entry points.
An animal showing these signs needs immediate veterinary care, not a tablet from a cabinet. The FDA's guidance is explicit about seeking veterinary attention for infested animals. A tablet may be part of what a veterinarian uses, but wound care and assessment of tissue damage are not things an owner can manage alone.
Owners should also resist the assumption that a preventive tablet removes the need for wound care. The parasite gets in through broken skin, so keeping wounds clean and covered, checking animals daily during travel to affected regions, and treating minor injuries promptly remain the first line of defense. A preventive medication is an addition to those habits rather than a replacement for them.
Human infestation is rare, and recent U.S. human cases have been travel-associated, including a Maryland resident who had returned from Central America. People are not the primary concern here, but anyone who develops a non-healing wound with visible larvae after travel to an affected region should seek medical care and mention the travel history.
