Martin set off from Bournemouth, taking a ferry to France before driving through the Netherlands and Belgium.
He then took his van through Germany and Poland, arriving in Krakow on Monday morning.
He told GMB viewers: "I'm heading today to the border, 20 kilometres from where the shelling happened yesterday.
"So I'm filled with a great sense of purpose but also a little bit of trepidation."
Richard Madeley said: "I bet you are"
Martin then went on to tell viewers what he was taking with him to the border.
His van, he said, was full of things from medicine for children, to respirators for new-born babies, saline drips and pet food.
Martin has made the journey through a charity called "Help from Bournemouth to Ukraine"
Posting on Facebook on Monday, the charity said: "With a pleasure I would say big thank you to all people, who are with Us and our Army of Hope"
Viewers were quick to praise Martin.
Taking to Twitter, William Smith said: "Well done Martin Roberts. Martin here is up 'n at 'em"
Aaron Howlett said: "Hats off to Martin Roberts... what a nice fella."
