Investigators find four faults in train safety system involved in fatal Wales crash

Four faults were detected in a safety system for a train involved in a fatal crash in mid-Wales, investigators have said.

A man was killed and a further 23 people received minor injuries when the two trains collided. The incident involved the Transport for Wales 6.31pm service from Shrewsbury to Aberystwyth and the Transport for Wales 7.09pm service from Machynlleth to Shrewsbury.
