Family wants answers to dad-of-one's death after Pets At Home tragedy

Furious relatives of a 39-year-old dad are demanding answers over their loved one's death four days after an incident at a Pets At Home warehouse.

Investigations are continuing into the tragedy which left forklift truck driver Shaun Potter, from Middleport, Stoke-on-Trent, in hospital with a 'brain injury' following an incident at the retail giant's Stoke warehouse on October 4.

He lost his fight for life at the Royal Stoke University Hospital four days later.

His cause of death was given as 'hypoxic brain injury and accidental mechanical asphyxiation and entrapment,' reports StokeonTrentLive.

Brother Wayne, 45, said after a pre-inquest review: “All his family and friends miss him so much, we can’t believe he has gone.

"It still hasn’t sunk in. It's a sad loss and he was so young. He’s left behind his daughter too.

“It’s been hard for us. It’s a huge loss. We are hoping to get some closure over what happened. We want to know whose fault it was, did they cut corners with health and safety? We need that closure to be able to put it to rest.”

The pre-inquest review also heard from the Health and Safety Executive (HSE).

A representative said: “Our investigations have not yet concluded. The investigation is looking into the health and safety and whether there were any breaches.

“We are fully aware of everything that’s gone on. We are waiting on a key piece of information - a report from the HSE specialist expert. There might be separate offences that we take to a criminal court.”

A jury inquest will be held into Shaun's death on May 30 and 31.
