A series of failures which led to the death of Jade McGrath, 19, after she was admitted to New Craigs Hospital in Inverness, have been recognised after the hospital formally apologised to her mother.
Jade had threatened to take her own life before she was allowed to walk out of the highland hospital unchallenged. Staff members seemed to fail their duty of care by not reporting her missing after she disappeared for over 45 minutes.
They also failed to inform officers that the 19-year-old was a suicide risk. Tragically it took 11 days for her body to be found by children who were playing less than a kilometre from the ward.
