
Engineering flaws led to the implosion of a submersible which killed five people near the wreckage of the Titanic, a report has found.
US agency the National Transportation Safety Board (NTSB) said Titan owner OceanGate’s engineering process for the sub was “inadequate and resulted in the construction of a carbon fibre composite pressure vessel that contained multiple anomalies and failed to meet necessary strength and durability requirements”.
