No one has heard from the athlete, who has also starred on X-Factor Celebrity and Celebs Go Dating since his disappearance, and his mum Julie has appealed for information.
Julie told Sun on Sunday : "Levi's family and I are very worried about him and desperate to know that he is OK.
"Levi, please get in touch with us or if anyone knows anything please contact us.
"Nobody has heard from him since Oct 29th when he was last known to be in The Old Irish Pub in Barcelona."
It is understood that worried relatives have reported Levi as a missing person to Spanish police.
Levi, who is currently playing for Worthing Raiders in the National League 2 South, started his career in 2017 at Bath before joining Ealing Trailfinders in 2020, initially on loan.
He became the first professional rugby union player to come out as bisexual in 2020.
Levi revealed he told his teammates at Bath about his sexuality via a message on a group WhatsApp chat.
It read: "I want to be open and honest with you boys, as friends and teammates. I'm bisexual.
"I have known since I was 18."
In 2019, the sports star performed on X Factor: Celebrity with fellow rugby lads Ben Foden and Thom Evans as Try Star.
He performed a saucy stunt, stripping down to just his trousers, on the ITV programme.
