He was given a second chance at life when a rescuer saved him from a kill shelter.
Now tiny Tom needs the public's help to regain strength and travel overseas to find his forever family.
Dog charity Cavaliers In Need is desperately pleading for donations to transport Tom from Turkey to Northern Ireland, Belfast Live reports.
Little Tom was discovered in a Turkish kill shelter at a dangerously thin weight with no hair on his body.
Looking at pictures, it's incredibly hard to believe the emaciated dog is actually a cavalier King Charles spaniel.
Charity workers hope in a few months' time, Tom will be strong enough to make the journey to a safe foster home and be a normal spaniel once more.

Cath Southwell, founder of the charity, said: "We're inundated at the moment, with 93 dogs in foster homes and 13 newborns being looked after by volunteers.
"In fact we've never had so many dogs in our care and the situation in Northern Ireland is getting worse wit dogs being surrenders or abandoned.
"We rely heavily on our foster care families but we are physically at our limit. Despite the pressures we have little Tom on our minds.
"If we think we have problems here, he is one of more than 1,000 dogs being cared for in Turkey at Emre's Garden, a shelter run by Emre Denir.
"The problem with stray and abandoned dogs is overwhelming in Turkey and new rules brought in by the government mean that native breeds are not allowed to leave the country.
"Dog shelters are filling up and dogs like Tom are getting squeezed with many ending up in kill shelters.
"Emre goes to the shelters and brings out the dogs he feels he can help get rehomed and Tom is one of them.
"We know nothing about his history apart from the fact he was found in a kill shelter and was probably taken there after being found in the streets.
"He was desperately thin and in need of intensive care and now he is safe with a vet where is is on a drip for nutrition and fluids and our plan is to help get him better and bring him to Northern Ireland.
"He needs to get a lot stronger before he could be expected to travel. But he will be matched with a foster family here and then we will find him a family to adopt him.
"We believe he is about four or five years old and we want to try to give him some good years and let him know love.
"So we desperately want to help him and bring him home and are asking people to help in any way they can.
"Our funds have been stretched in recent times due to some very expensive heart and orthopaedic surgeries and treatments or anything that anyone can spare will help us to save him.
"Anything that might be left over will be used to help other dogs no nothing will be wasted. The need is great at home and abroad and we are doing our best.
"We just hope little Tom will gain enough strength very soon so he can start his life again and know what it feels like to be loved, cherished and held safe in someone's arms."
