Hundreds of NHS patients have suffered serious, preventable harm, including operations on the wrong body part and surgical items left inside them, according to new data revealing 403 "never events" in the past year.
These serious patient safety incidents, termed "never events" because they are considered entirely preventable, highlight critical failures within the health service. The annual figures from NHS England cover the period up to March this year.
