Paul Alexander earned a law degree, practiced law, wrote a memoir, and inspired millions through his determination to live independently despite spending most of his life inside an iron lung. Martha Lillard similarly demonstrated remarkable resilience, adapting to life with significant physical disabilities while outliving expectations for someone who experienced severe paralytic polio.

Polio: An Old Deadly Disease That Took the Lives of Many

According to the U.S. Centers for Disease Control and Prevention (CDC), poliomyelitis (polio) is a highly contagious viral infection caused by the poliovirus. It primarily spreads through contact with contaminated food, water, or surfaces and can infect the nervous system, sometimes leading to permanent paralysis.

Causes

Polio is caused by infection with the poliovirus, which spreads mainly through the fecal-oral route and, less commonly, through contaminated respiratory secretions.

Symptoms

Most infections produce few or no symptoms, according to the Cleveland Clinic. The clinic has also noted that symptomatic illness may include:

  • Fever
  • Fatigue
  • Headache
  • Sore throat
  • Nausea or vomiting
  • Neck stiffness
  • Muscle pain

In a small percentage of cases, the virus attacks the spinal cord and brainstem, causing muscle weakness, paralysis, and, in severe cases, respiratory failure requiring mechanical breathing support.

  • Laboratory testing using polymerase chain reaction (PCR)

Treatment

There is no cure for polio once infection occurs. Treatment focuses on supportive care, including:

  • Long-term mobility assistance

Patients whose breathing muscles became paralyzed historically depended on iron lungs to survive.

What Is an Iron Lung?

According to Encyclopaedia Britannica, an iron lung is a negative-pressure ventilator that encloses most of the patient's body while leaving the head outside. By rhythmically changing air pressure around the chest, the machine helps draw air into and out of the lungs, effectively replacing the work of weakened respiratory muscles.

Although largely obsolete today because of modern ventilators, iron lungs saved countless lives during the height of global polio epidemics.

Risk Factors and Prevention

Before widespread immunization, children under five years old were among those most likely to develop polio. The greatest protection remains vaccination, which has reduced global polio cases by more than 99%, according to the World Health Organization (WHO), since international eradication efforts began.

Preventive measures include:

  • Routine childhood polio vaccination
  • Maintaining high vaccination coverage
  • Good sanitation and hygiene
  • Public health surveillance to rapidly identify outbreaks
