The ACT Coroner is looking into the death of a five-year-old child at the Canberra Hospital earlier this month.
Sources have told the ABC that the girl was taken to the hospital on July 4.
It is understood a number of tests were carried out over several hours.
A decision was then made to transport the child to Sydney for treatment but she died before this could happen.
The ABC understands the family was concerned about how the situation was managed.
It is not clear yet when or if there will be an inquest.
It is also unknown why the girl died.
In a statement, Canberra Health Services (CHS) confirmed the death had been referred to the coroner.
"We are also ensuring that team members who were involved in the child's care are receiving support.
"The child's death has been referred to the coroner and CHS is unable to provide any further comment."
